How do you order: There are three routes to applying for a Birth or Death Certificate: 

  • Online : Time Frame: 

Birth Certificate: Approx. 4-6 weeks;

Death Certificate: Approx. 4-6 Weeks

  • In-person: Time Frame:(fastest)

Birth Certificate:less than 2 weeks; 

Death Certificate less than 3 weeks

Cost: $35 + $12 Shipping

**If you need a birth certificate quickly, make your request in person at our Local Vital Records Offices.  

***If the birth occurred more than 80 years ago, it is considered a Genealogical Request and cannot be ordered online. For more information see the Vital Statistics & Registry Website.

Requirements for ordering:

  • You must complete the following requirements to receive a death certificate.
  • All requests must include a copy of the requestor’s valid identification, payment of the appropriate fee and proof of relationship to the individual listed on the vital record.

Who can apply for a birth or death certificate?

  • A State or local Registrar may issue a certified copy of a vital record only to”
  •  persons who establish themselves as the subject of the vital record
  •  the subject’s parent
  •  legal guardian or legal representative
  • Spouse
  • Child
  • grandchild 
  • sibling, if of legal age
  • to a State or Federal agency for official purposes, pursuant to court order or under other emergent circumstances as determined by the Commissioner.
  • All other applicants will be issued a Certification that state the document is not for identification or legal purposes.
