How do you order: There are three routes to applying for a Birth or Death Certificate:
- Online : Order A Record Online from www.VitalChek.com. Fees are $60.00: $34 for the record, $12 for the RUSH fee, and $14.00 for the VitalChek processing fee. This is a two-week processing time. There is an optional overnight delivery fee of $19.75, for a total of $79.75, and you should receive the record in 1-3 business days.
- In-person: In-person ordering is available by appointment only. Dates and Times are limited to Tuesdays and Thursdays, 10:00am - 2:00pm. Schedule an appointment.
- By Mail: Order A Record by Mail Via US Postal Service by downloading and completing the application from this website. Mail the completed application, a photocopy of your valid ID (if applicable), and the fee to the PO Box address listed on the application. Certified copies of vital records are $34. Optional RUSH fees and processing times are listed on each application.**
Cost: $60
The State Vital Records Office is closed on recognized state holidays.

- Will Call:
Will-Call pick-up is available by using VitalChek. Order online and pick-up your Vital Record within 2-5 business days. - Social security number
- Valid photo ID
- Correct answers to related question
- A written statement of identity.
Who can apply for a birth or death certificate?
- A State or local Registrar may issue a certified copy of a vital record only to”
- persons who establish themselves as the subject of the vital record
- the subject’s parent
- legal guardian or legal representative
- Spouse
- Child
- grandchild
- sibling, if of legal age
- to a State or Federal agency for official purposes, pursuant to court order or under other emergent circumstances as determined by the Commissioner.
- All other applicants will be issued a Certification that state the document is not for identification or legal purposes.
